Camera - required. The camera is the light sensor: iOS exposes no ambient-light reading to apps, so the exposure is derived from the ISO, shutter and aperture the camera reports for the scene you point it at. Frames are read for those values and discarded. Nothing is photographed, recorded, or saved to your photo library.
Microphone - Shutter Check only. Fire a mechanical shutter at the phone and the app listens for the open and the close to time the speed the camera is actually giving you. iOS asks for the microphone the first time you use that feature, never at launch, and no audio is recorded or saved.
No location, no photo library, no contacts and no notifications are requested, and the app contains no networking code at all. The App Store privacy label is Data Not Collected.

Only for Shutter Check, the feature that times a mechanical shutter by listening to it open and close. iOS asks for the microphone the first time you use that feature, not at launch, and no audio is recorded or saved. If you never use Shutter Check, you will never see the prompt, and the rest of the app works unchanged if you decline.
Yes, always. There is no networking code in the app, so every part of it works in airplane mode. There is no account to create and nothing to sign into.
On your iPhone, in a single file inside the app. Your camera profiles, calibration and logbook never leave the device. The App Store privacy label for the app is Data Not Collected.
Light Meter₊ is free, with one optional in-app purchase - Light Meter₊ Full, $6.99 one time. It is not a subscription and does not renew. It is tied to your Apple ID, so you can restore it on your devices at no extra cost, and it supports Family Sharing. Apple handles the payment; we never see your payment details.
